Italy is a relatively small country compared to America, so they can transmit shows across the country with only a few stations. The United States cannot, therefore broadcast channels need local stations in each county/state so their shows can be broadcasted across the country. Also there's thousands of cable channels and a small number of local stations. NYC only has 15 local stations.

But they could still put the cable channels as digital subchannels or is there a problem?

The problem is that the cable channels earn millions of dollars in carriage fees that wouldn't be recouped by going OTA.

Well, they could create their own or collaborate with an advertising company to manage commercial breaks, thus making money.

After all most cable channels are owned by big companies, so there shouldn't be a financial problem.

Only a few stations? the first nine channels in Italy are all national, excluding the Rai channels and Telemontecarlo (later La7), every national channel in the 80's used to have local stations because until 1990, they weren't recognized as national channels.

Most of these stations were O&O, had names instead of callsigns, and sometimes even local programming.

Italy can broadcast all around regions thanks to satellites, why can't the US do the same?

If local programming and news are a problem, they could make multiplexes (or virtual channels) containing both one of the fourth networks (with local programs but without the four letter callsigns) and the cable channels it owns, like Rai does with Rai 3 in Italy.

Italy can do it because they're a small country. America cannot because they're a big country.
