Question about Simpsons replacement form

I just printed out the Simpsons S6 replacement case form, but I'm a little confused. Do I send them the slip of paper I printed out with my address on and their instructions inside the envelope, or do I tape their address and my address thing onto the envelope?

i don't remember but it was pretty self explained...

you print out the little .pdf file they give you, then a proof of purchase aka ticket or different cover sleeve or cut the proof of purchase,

i just sent the "get a different cover" paper, i put the .pdf file and the cover paper on an envelpe and mailed it to were i needed to.

I got a confirmation that it was sent to me and should arrive soon, that was almost 2 weeks ago thoug.
