I got this idea after watching some YouTube videos (Primarily Akom's G1 work) and as I couldn't find a topic for it. I decided to start one myself.
The way it'll work is this. you can either post an image or talk about the error in question (or both).
I'll start off with this infamous bit from the Five Faces of Darkness part 3 from G1:

Now you're wondering what is wrong here, right? well to run it down:
Snarl is on the Decpticon side and is in Rodimus' colors (I have since named him Rodimus Snarl.
Octane, Motormaster and a few others are wildly colored (with Octane looking like a Dinobot).
In fact the only ones not looking horrid are Vortex and Dead End (and still they look off model somewhat).
Now let's see your findings.
The original TMNT show sometimes got the turtles' voices mixed up. For example, in the first episode, Turtle Tracks, there is a scene that has Donatello saying with Raphael's voice. "Ain't you up on current events, lady? We just saved your life!"
There are probably other examples of this, but I can't remember them at the moment.

Transformers has so many animation errors, it's almost impossible to find them all. I like to play "find the Starscream" when watching my DVDs because the guy seems to pop up everywhere.
This picture from Naruto is one of my favorite animation goofs. No, the kid ninja didn't get his head chopped off, it's just...missing.

You know that animation error from Challenge of the Superfriends where it looks like Green Lantern suddenly grew a third arm? I dismiss it as a ring construct.
Not so easy to do with stuff like Batman's changing Bat-symbol colors, though.

Not to mention that in the intro sequence, in the scene where theyre marching, GL is drawn with darker skin. Pokemon doesn't get this sort of thing nearly as much as you'd expect, and although not as bad as a misplaced head, it's still pretty funny.
(The second image is a bit blurry, but just to clarify; Yes. They did manage to somehow have an upside-down Meowth mouth appear instead of Wobbuffet's for that frame.)

Apparently there were enough mistakes in the original TMNT cartoon show that several sets of them (http://www.youtube.com/view_play_list?p=2F0516B328BD63DA) have been compiled on the net.
If I had a dollar for every animation mistake in the first two Super Mario cartoons, I'd be rich. Wrong voices coming from characters' mouths, Mario and Luigi's colors being switched around...both shows, while entertaining back in the day, are absolute messes animation-wise.

I recall some episodes of G.I. Joe where a character is seen both out in the field and back at The Pit at the same time.
And yeah, I remember a lot of coloring/vocal mistakes in Ninja Turtles. As a kid, I used to be confused as to why Leonard would talk with Raphael's voice, or why Michaelangelo would suddenly be wielding sais in one shot.
I also remember a moment in Batman: Mask of the Phantasm, an over-the-shoulder shot of Batman, where he says (I think) the line, "...so you could switch the blame to your father if you had to," and his jaw stops moving about a good second or two before he actually finishes talking.
There was an early episode of Code Lyoko (i think a season 1 episode) where they had Yumi's voice speaking with Ulrich's mouth moving.
Can't really think of any others.

In the Gargoyles episode "Protection", Elisa wears a... revealing outfit. But for about one second in Dracon's room, it changes back to her normal black t-shirt and red jacket.
And... Transformers! More!

Well, I'm watching the fourth episode of Gurren Lagann and I can see why this episode gets as much criticism as Akom does for G1. It looks off by quite a bit. Characters are drawn with different mouths, different eyes, slightly choppier animation and still it looks better then Carnage in C-Minor (which is quite possibly the worst episode in any series animation wise) Heck, Akom wasn't even trying that episode, what with these mistakes:

Seriously, the above picture is not excuseable for anyone, the writers, producers, Sunbow, Akom or otherwise. That is just epic fail.

Speaking of G1 Season 3, am i the only one who thought it had a "blocky" look, as opposed to the first two seasons? And speaking of animation errors, dont forget sideswipe at decepticon HQ in "The Secret of Omega Supreme". And while we're at it, there was a scene in GI Joe where Duke jumped out of a plane and for a split second, inexplicably had facial hair. One of the animators must have been bored.

I remember there being a lot of animation errors in the original Super Dimension Fortress Macross TV series (first released as Robotech in the US). Some were more noticable than others. A few recently were fixed on the Japanese Blu-ray restoration.

There was a weird one that I have never seen repeated in any series. thundercats: Trouble with Thunderkittens. A weird half opaque black smudge mark throughout the entire show...

One of the most infamous flubs in Transformers G1 was Frank Welker doing his Dr. Claw voice on Soundwave in that one episode.
Daria had some flubs as well, but I'll post the links later.
When Jubilee is captured by the Sentinels in pt. 1 of the Fox Kids' X-Men pilot, pt. 2 has her in the Xavier School with the other X-Men. Here's the flaw: Jubilee was still captured until the end of the episode.
Just found another small one. My brother was watching Little Bear, and in the episode 'Cat's Shortcut', there's a scene towards the end where Cat's eyes are white instead of their normal green.
Another one I remember is how Godzilla would constantly change size in the Hanna-Barbera series. There were plenty of scenes where, for example, a boat would be larger than his head, then in the next shot it fits in the palm of his hand.
Also, in the JLU episode 'Dark Heart', Wildcat is shown in the background of a few scenes tearing apart robots with claws that he's not supposed to have.
