Trying to collect Batgirl (Cassandra Cain)

I just wanted to know a few things that would help me with this collection:

What comics (and what issues) did she appear in (as Cassandra Cain and/or Batgirl) before she got her own comic? And are any of her appearances in trade paper-back form?

How many TPBs does her comic have, and what issues were contained in them?

Aside from Teen Titans, what comics (please include issue #'s as well) has she appeared in since her title ended?

A while back -- over a year ago, it looks like! -- I was in a thread on another forum where we tried to sort out the question of how much of Cassandra's "Batgirl" series has ended up being reprinted in TPBs.

I don't claim to know every issue of every title that she ever appeared in, but I'll try to cover the bare essentials of where she started and how much of her solo title has been reprinted.

The year-long "Batman: No Man's Land" event has been collected in five TPBs. It included issues of several different titles. Somewhere in Volume 3 is Cassandra Cain's first appearance as a civilian. I believe it's in Volume 4 and Volume 5 that she starts wearing the Batgirl costume after Batman gives it to her and says something like "It is a sacred trust." (Although that could have happened at the end of Vol. 3 -- been a very long time since I reread my copies).

Then came her solo series. It went up to "Batgirl #73." I'm told that the TPB collections go like this:

Batgirl: Silent Running ("Batgirl" #'s 1-6)
Batgirl: A Knight Alone ("Batgirl" #'s 7-11, 13-14)
Batgirl: Death Wish ("Batgirl" #'s 17-20, 22-23, 25, and "Batgirl Secret Files No 1")
Batgirl: Fists Of Fury ("Batgirl" #'s 15-16, 21, 26-28)
Robin/Batgirl: Fresh Blood ("Batgirl" No 58-59, Robin No 132-133 -- it was a crossover with the story going back and forth between the two titles)
Batgirl: Kicking Assassins ("Batgirl" #'s 60-64)
Batgirl: Destruction's Daughter ("Batgirl" #'s 65-73)
Batgirl/Ghost: The Resurrection Engine (Collecting a four-part miniseries that was a DC/Dark Horse crossover)

And "Batgirl #24" ended up being reprinted in the Batman TPB Bruce Wayne: Murderer? since it was part of that crossover that ran through several titles at once.

Now I've got to run off to supper, but I think that covers -- pretty well -- which of Cassandra's Pre-OYL appearances (as Cassandra and then as Batgirl) ended up in TPB collections.
