I suppose this topic depends on your defintion of "deadly" and "arch villain." Is the topic meant to suggest that all super-powered villains that have appeared in the pages of Flash over the years are arch villains, or just the ones that have appeared over and over again?
Personally, I feel that Batman80's answer of "Reverse Flash" is, by all logic, an excellent answer. However, is the Reverse Flash considered deadly for having the gaul to murder, or does your labelling of him extend to some further evidence?
I will admit to you right now that I am not an expert on the Flash. I know enough about him to have an sort of conversation, but I will not pretend to have read all of Reverse Flash's past appearances, nor I will I claim to understand all of the powers of all the other Rogues.
Having said that, I think Flash's most deadly villain is Replicant. Replicant, whose origin is told in his first apperance in the fairly recent Flash Secret Files #2 (November 1999), is the "nephew" to all the original Rogues, who he has adored and aspired to be for most of the years of his life.

Consider the possibilities of Replicant's powers: he absorbed the powers of Heat Wave, Dr. Alchemy, Weather Wizard and Mirror Master. With such powers, he could very well do most anything. It's just a question of getting a competend writer to handle him in a serious story. It would certainly make him a larger threat than the many other Flash villains.

I, myself, won't get into him and his possibilities as a character, but judge for yourselves. :)
