Tendo Dokuta (http://www.spike.co.jp/tendo-dokuta/)
Out of all of the games I've seen so far, this game has the best use for the pen.
You are doctor that graduated at the top of his class and was stationed in one of the biggest hospitals in Tokyo. The game's setup is supose to be similar to your average pick and choose game. However there is a deeper side to this involving a very huge cast of people you have to help. Patients can range from simple chest pains to broken bones to even bullet wounds.
Using the stylus pen, your job is to find out the source of the problem and use it to either heal, mend, or cut open for an operation. The lives of the people depend on your perfection when using the pen. One wrong move and it's game over.
There is a game trailer and a quick demo on the site.
